The following handouts are used at Inlet Birth as part of the informed choice process. We will review them with you during your care with us. Feel free to ask us questions about your care at any time.
Important note: during your journey with us, fillable handouts are e-mailed to you by your caregiver so that you can fill them out electronically, after which they are automatically uploaded to your chart. If you want to fill out the PDFs below, please download them to your computer (rather than your phone or tablet), save, fill out and ‘sign’ electronically, and then e-mail them back to us as attachments.
